Health Permit Applications
Apply for a Health Permit
If you plan to sell, distribute, or store food in the City of Mansfield, you must apply for the required permit(s). You are also required to receive a valid health permit to operate a child care facility, or a public pool, spa, or public interactive water feature (PIWF).
If you are applying for a permit renewal, a digital payment link may have already been sent to the contact information provided on your previous application, please check to ensure you have not already received it. Permit fees must be remitted to the office prior to or on the invoice due date. If you do not pay by the invoice due date, you may be subject to late fees or at risk of your permit being denied or suspended.

Registered Pool Operator
After successfully receiving credentials from an accredited program, apply to register your Pool Operator Certificate with the city.

Registered Food Manager
After meeting the Texas Department of State Health Services requirements, apply to register your Food Manager Certificate with the city.

Reinspection Application
Apply for a health inspector to perform a new, pre-operational inspection or a re-inspection of your establishment.

Seasonal Food Establishment
Apply for a Seasonal Food Establishment Permit for food establishments in a fixed location for longer than 15 days but less than one year. This can include Farmer's Markets and other seasonal establishments, such as concession stands and snow cone stands.

Temporary (Event) Food Service
Apply for a Temporary Food Establishment Permit (in conjunction with a Special Event Permit) for food establishments in a fixed location for up to 14 days.
